Please follow the steps below:

  1. Read JPPM Service Agreement (Click here to download) (Click here for Easy Read download) – Do not fill in this form. – Once we receive your sign up webform, we will forward a service agreement to your email for you to sign and return to us – please let us know if you need assistance reading the agreement.

  2. Provider Relationship Endorsement – must be completed
    Please Call NDIS 1800 800 110 and request a provider relationship endorsement for provider number: 4050123867
    .
  3. Fill and submit – Participant Details Form (web sign up form below)

  4. Your plan management portal – Once plan management set up is complete, you will receive an email with a link to register your login and set up to your smart device – Please head to the INFO LINKS tab on our website for instructions on setting up for Android and iPhone
